following forms as part of the mandatory paperwork. Please note that these forms are
editable PDF files, fill in the information and print out:

Mandatory Paperwork
•Child must be present for weigh-in at registration
•Completed Participation contract (Print above form)
•Original Birth Certificate (2 copies for GLM to keep)
•Proof of Residency (SDG&E bill, phone bill, etc.) (1 copy for GLM to keep)
•Proof of Medical Insurance (2 copies of Policy# or ID#)
•Physical form completed by licensed Doctor (Print above form)
•Year-End School Report Card (2 copies for GLM to keep)
•Recent Picture (for GLM to keep)
***Failure to provide any of the above documentation prior to the 1st
day of practice
will result in your child being ineligible until paperwork
is complete
